Kustom Rehab is an out-of-network provider for all insurance plans. We will provide you with an invoice (superbill) at the end of each week that you can submit to your insurance company if you choose, with varying re-imbursements to you based on different plans.
To understand your out-of-network benefits, please ask your insurance provider:
- What type of health plan do I have?
- What does the policy cover and not cover?
- How much does the plan cost?
- How is the policy premium determined?
